Major complications are less common and include:

Bleeding. It is the most common serious complication, ranging from 1.7% to 0.062%.

Puncture of other organs. It is the second complication in order of frequency. The puncture of the lung, expressed by a pneumothorax, occurs with a frequency between 0.55% and 0.35%. Also described the puncture of other organs such as kidney, colon and exceptionally pancreas, adrenal glands and small intestine. Most punctures of these bodies are completely asymptomatic.

Biliary peritonitis. Its frequency is 0.22% and is due to puncture of an intrahepatic bile duct, the bile duct or gallbladder.

Arteriovenous fistulas. They are produced by the puncture reaching hepatic artery branches and venous branches. Are usually asymptomatic and tend to close over time.

Infection. Secondary bacteremia is relatively common, especially in patients with cholangitis, being much rarer the occurrence of sepsis.

Tumor spread. Rare, and always secondary to the puncture of tumors.
